Get Notice: Undefined index: in manage.php

WHen trying to create a gallery–add a name, description, path and author, I get these errors. This is my favorite slide show plugin–my client loves it–went to another one because I can’t figure this out. Help is appreciated! http://www.REACHPikesPeak.org. THANKS!
